Privacy Policy
Your privacy is important to Life Options. We understand that health is a personal, private subject, and we want you to feel comfortable visiting our website and using its services. This privacy policy will tell you what information we collect, how we protect it, and what choices you have about how that information is used. We urge you to read our privacy policy so that you will understand both our commitment to you and your privacy, and how you can participate in that commitment.

PLEASE NOTE: Any information (including personal and medical information) that you reveal in a public forum (a bulletin board, mail list, chat room/event) is not subject to this privacy policy. This information will be seen by third parties outside of Life Options and may be used by them to contact you, or for unauthorized purposes. If you link to a third party site from Life Options, any information you reveal on that site is not subject to this privacy policy. You should consult the privacy policies of each site you visit.

Personal information we collect
We ask for information in several areas of the Life Options website: materials order forms, Kidney School, the Showcase of Ideas, and the Patient Scrapbook. Information we request may include your name, address/phone number/email, age, cause of kidney problem, years of education, and your attitudes or opinions about Life Options, Kidney School, or both. Required information is indicated by an asterisk*. Provision of all other information is optional.

We also collect information about you and your use of our site through the use of Cookies. "Cookies" are small computer files that we transfer to your computer's hard drive that allow us to know how often someone visits our site and the activities they conduct while on our site (such as the chatrooms you visited, etc.). This information helps us to monitor how many people are using our site and for what purpose, so we can assess the impact of the Life Options program, and adjust our activities to better suit the needs of patients and professionals.

How we use your information
When you contact Life Options, we enter your name and identifying information into our private database to allow us to track requests and trends. Life Options does not release information gathered on the Life Options website to any outside parties. Your name and information will never be sold; we will contact you only if you indicate a willingness to talk to us. If you make a comment, it will not be connected with your full name or identifying information.

Survey and evaluation information (e.g., at the end of each module of Kidney School) is put together with the responses from hundreds of other people and used for research purposes, to help us improve our efforts.

Legal disclaimer

The information you find on the Life Options website, including information in Kidney School, is provided "as is," and to the extent permitted by law, the Life Options Rehabilitation Program, the Medical Education Institute, and Amgen Renal Advances and their affiliates, employees, officers, board of directors, or shareholders give no warranty of any kind, either express or implied, including without limitation any warranties of condition, quality, performance, merchantability, fitness for a particular purpose, or non-infringement in relation to information, services, or products provided through or in connection with the Life Options website.

To the extent permitted by law, the Life Options Rehabilitation Program, the Medical Education Institute, and Amgen Renal Advances disclaim all liability for any damages or injury caused by any failure of performance, error, omission, interruption, deletion, defect, delay in operation or transmission, computer virus, communication line failure, theft or destruction or unauthorized access to, alteration of, or use of record, whether as a result of breach of contract, tortious behavior, negligence, or under any other cause of action.

The contents of the Life Options website pages, including, but not limited to text, graphics, and icons, are trademarked materials owned or controlled by the Medical Education Institue, Inc. and Amgen Renal Advances. No permission is granted here to you to use, or permit others to use, Life Options' icons, site address, trademarks and service marks, or other means to hyperlink other Internet sites with any page in the Life Options website, and the Medical Education Institue, Inc. and Amgen Renal Advances assume no responsibility for any other party's site hyperlinked to the Life Options website or in which any part of the Life Options website has been, or may have been, hyperlinked.
